Project Description

The HCH Program of the City of Portland is operated by the City of Portland's Public Health Division of the Department of Health and Human Services.

Services

The HCH program provides on-site primary health care, dental services, and mental health and substance abuse services. Extensive outreach is done by primary care, dental, and mental health and substance abuse staff at meal sites, shelters, half-way houses, and campsites. Intensive mental health outreach and intervention is provided through collaboration with the Maine State Department of Health and Human Services. Respite care provides 24hour nursing care. Access to volunteer sub specialists is provided. Primary care and dental services are available to teens in special clinics. All adult ambulatory care services are available to clients regardless of their ability to pay.

Linkages

The Division of Social Services is part of the City of Portland's Department of Health and Human Services.
