Project Description
The HCH Program of the City of Portland is
operated by the City of Portland's Public Health
Division of the Department of Health and Human
Services.
Services
The HCH program provides on-site primary health
care, dental services, and mental health and
substance abuse services. Extensive outreach
is done by primary care, dental, and mental
health and substance abuse staff at meal sites,
shelters, half-way houses, and campsites. Intensive
mental health outreach and intervention is provided
through collaboration with the Maine State Department
of Health and Human Services. Respite care provides
24hour nursing care. Access to volunteer
sub specialists is provided. Primary care and
dental services are available to teens in special
clinics. All adult ambulatory care services
are available to clients regardless of their
ability to pay.
Linkages
The Division of Social Services is part of
the City of Portland's Department of Health
and Human Services. Their services include shelters,
employment and youth services, housing placements,
general assistance, short term crisis intervention,
and day respite services. Mercy Hospital contributes
ancillary services, such as lab work, EKG, EEG,
and x-rays. The Portland Community Free Clinic,
another public health program, is supported
by a unique combination of community volunteers,
Mercy Hospital, and Public Health Division.
This clinic offers specialty services, such
as podiatry, orthopedics, and dermatology clinics.
A volunteer dental clinic, another public health
program, provides limited dental services. A
clinic serving runaway and homeless youth operates
within a collaborative of youth-serving agencies
at a multi-service site. The community resource
center is a centralized site of services.

Project Description
This project is located in the rural, southernmost
county in Maine. Dual programs, HCH and Public
Housing Primary Care, are located at a multi-services
site (WIC, Head Start, Outreach, Transportation,
etc.) and operated by York County Community
Action Corporation.
Services
Services include primary and preventive health
care, oral health, and mental health/substance
abuse diagnostic and treatment services for
adults, children, and adolescents in York County.
Services also include support/liaison services,
respite care beds, and access to transitional
and permanent housing. A Healthy Habits Center,
focused on implementing preventive health strategies,
is implemented collaboratively with York County
Shelter Programs.

Sanford Housing Authority, Counseling Services,
Sweetser Homeless Youth Outreach Services, Goodall
Hospital, University of New England Schools
of Medicine and Allied Professions, Maine Medical
Center, and Partners for Healthier Communities.
