Project Description

The mission of Health Care for the Homeless is to provide health-related services, education, and advocacy to reduce the incidence and burdens of homelessness.

Health Care for the Homeless provides comprehensive primary care, dental, mental health, social work, addiction, outreach and HIV testing, prevention and treatment services in Baltimore City. Weekend clinic sessions are available with a womens' only session in the morning and an afternoon session targeting day laborers and others unable to access weekday clinics. HCH also operates three housing first programs as well as a re-entry program. Through service agreements, many of these services are available in Baltimore, Frederick, Harford, and Montgomery Counties. Children and families are served through a service agreement with Mercy Medical Center.

Linkages include the National Health Care for the Homeless Council and 50 plus community coalitions including Maryland Advocates for the Poor, Maryland Medicaid Avisory Committee, Maryland State Interagency Council on Homelessness, Baltimore City 10 Year Plan to End Homelessness Leadership Group, Stop Homelessness and Reduce Poverty Coalition, Baltimore City HIV Commission, Physicians for a National Health Plan, Greater Baltimore Committee, Maryland Health Care Commission, and The Greater Baltimore Health Services Planning Council.
