Project Description
Baton Rouge Primary Care Collaborative provides
comprehensive health care for homeless persons
in the Baton Rouge Metropolitan area. The health
center was named in honor of the patron saint,
Saint Martin de Porres, who devoted his life
to relieving the suffering of the poor and infirmed.
It is co-located on the campus of the Society
of St. Vincent de Paul, a non-profit Christian
organization that has served the Baton Rouge
community for 140 years. St. Vincent de Paul
operates two homeless shelters for men and one
for women and children which is also located
on the same campus, as well as a Dining Room
which serves hot meals daily, a community pharmacy
that provides free medication, a dental program,
and free legal advice.

Baton Rouge Primary Care Collaborative has
linkages with numerous local health and social
service agencies, including Earl K. Long Medical
Center, Society of St. Vincent de Paul, Mays
Foundation, Capital Area Human Services District,
and the Capital Area Alliance for the Homeless.

Project Description
The New Orleans Health Care for the Homeless
Program (HCHP) operates a primary health care
clinic in the central business district and
an adolescent drop-in clinic in the French Quarter
area.
Services
Services include medications, x-rays, lab tests,
TB tests, STD diagnosis/treatment, immunizations,
dental, podiatry, HIV testing/counseling, health
and nutritional counseling, mental illness and
substance abuse diagnosis/treatment, case management,
outreach, clinic for homeless/at-risk youth,
and pro bono legal work. Contracted services
include lab tests and x-rays, substance abuse
services, outpatient counseling, case management,
housing referrals, job search, education, and
basic needs. Pharmacy, Early Periodic Screening
Diagnosis Treatment, ophthalmology, well child
care, emergency medical, Women, Infants and
Children program, perinatal/pediatric, and TB
prophylactic treatment are also provided.

HCHP has linkages with various programs, including
Unity for the Homeless, N.O. Mission, Volunteers
of America, Covenant House, Louisiana State
University Health Care Systems, Louisiana Office
of Public Health, and Integrated Mobile Assessment
Team.

Project Description
The DeSiard Street Clinic provides core primary
medical services, behavioral health services,
including substance abuse counseling and short
term therapy, access to dental services, and
referrals of chronically mentally ill patients
to the local community behavioral health network
of mental health providers.
Services
Primary medical services include preventive,
episodic, chronic disease and follow-up, basic
diagnostic laboratory services, limited pharmacy
services, case management for public insurance
and assistance, translation services, health
education, childhood immunizations on site,
Ryan White AIDS program services, breast and
cervical cancer screening, behavioral health
screening and assessment, and mental health
and substance abuse treatment. The site provides
limited laboratory services with referral to
a regional lab for complex laboratory work.
The site also provides referrals to radiology
services within the service areas of the charity
care hospital network, where low income patients
can qualify for charity care support as needed
and all forms of payment are accepted. The clinic
has extended hours as needed. The facility and
program are incorporated into the Primary Health
Services Center quality improvement program
as well.
