Project Description
Family Health Centers provides primary health
care and outreach services to persons who are
homeless in
Louisville and the surrounding areas. The Phoenix
Health Center is the home for the primary health
care clinic and
the base of operations for outreach services.
Primary care is also provided in five homeless
shelters serving women
and children.
Services
The Phoenix Health Center, located just east
of Louisville's central business district, provides
health care and
outreach services to men, women, and children
who are homeless. The health clinic operates
from 7:30 am to 4:00
pm, 5 days a week. The clinic staff includes
two nurse practitioners, a full-time physician,
three medical assistants,
and three front office workers. Services include
adult medicine, pediatrics, TB testing, HIV
counseling and testing,
mental health, laboratory, medications, referrals,
and transportation. The dental clinic operates
4 days per week, and
the staff includes a part-time dentist, part-time
hygienist, full-time dental assistant, volunteer
dentists, and dental
students. Health outreach is provided on-site
at five shelters that serve women and children.
Health outreach staff
consists of a nurse practitioner and a medical
assistant. Mental health staff include a social
worker and psychiatric
nurse practitioners. Social services provide
outreach and case management to disabled adults
who are homeless.
Two U.S. Department of Housing and Urban Development
(HUD)-funded social services workers provide
substance abuse counseling, referral, and case
management. A U.S. Department of Housing and
Urban
Development (HUD)-funded child advocate provides
outreach and case management with families whose
children
have special needs. Inpatient substance abuse
treatment and mental health counseling are provided
off-site, also
through U.S. Department of Housing and Urban
Development (HUD) funding.
Linkages
Essential linkages have been established with
various community agencies. The local community
mental health
center provides three mental health workers
at the Phoenix Health Center who provide outreach
and case
management services to individuals who are homeless.
Patients needing specialized medical services
are referred to
the University of Louisville's health care clinics
for further treatment. Other linkages include
Jefferson Alcohol and
Substance Abuse Center, Family and Children's
Counseling Services, Jefferson County Medical
Society, Kent
School of Social Work, Spalding University,
Legal Aid, Coalition for the Homeless, City
of Louisville, Jefferson
County Division of Community Development, and
local shelters and homeless service providers.

Project Description
Hazard Perry HCH is a partnership providing
outreach, housing, and health care for homeless
people in eastern
Kentucky. Primary care is provided through clinics,
volunteer physicians, and at area shelters.
Services
Primary care is available at Little Flower
Clinic and Clover Fork Clinic. Referrals are
made for specialty and
inpatient care. Lab and ancillary services are
available on-site. Pharmacy is provided on site
at primary care clinics.
Mental health and substance abuse treatment
are provided through Kentucky River Community
Care and
Cumberland River Comprehensive Care.

Project Description
The Powell and Estill County HCH Project is
a partnership providing participant recruitment
and retention, housing
and supportive services, and primary health
care for homeless persons in two rural Appalachian
Kentucky counties.
Primary care is provided through a mobile health
clinic and at a rural health clinic.
Services
The project provides the target population
with access to a full range of primary, preventive,
enabling and
supplemental health services, including oral
health care, mental health care, and substance
abuse services, either
directly or through established arrangements
and without regard for ability to pay. Primary
care is available through
a mobile health clinic with rotating clinic
sites in Powell and Estill Counties. Referrals
are made to a rural health
clinic for conditions regarding higher levels
of care and to other sources for specialty and
inpatient care. Lab and
ancillary services are available on the mobile
clinic and at the rural health clinic. Mental
health and substance
treatment are provided through local community
mental health providers.

Project Description
The Hope Center provides emergency shelter,
food, and clothing to people who are experiencing
homelessness. The
Hope Center is a multi-location, comprehensive
social service agency delivering a variety of
programs to help
people get off the street and stay off the street.
Services
Health care is provided at the Hope Center.
The clinic is open Monday through Friday, and
the staff includes a
nurse practitioner, staff assistant, and social
worker. Services include diagnosis and treatment
of medical conditions,
lab tests, x-rays, basic medications, and referrals
for substance abuse, mental health, and social
services/case
management. Outreach services by the mobile
van provide health education and wellness education.
Linkages
Access to emergency health care, mental health
services, and inpatient hospital care is facilitated
through the
Lexington Fayette County Health Department Primary
Care Centers existing network. In addition to
collaborative
efforts in providing primary care, clients can
be referred through cooperative arrangements
for specialist care
including dental, ophthalmologic, substance
abuse treatment, mental health counseling, personal
needs, and other
social services.
