Project Description

ECHO Community Health Care (ECHC) provides services to persons who are homeless, uninsured, or underinsured at three sites. The Health Care for the Homeless team consists of full time employees, including a physician, a family nurse practitioner, five registered nurses, two social workers, a homeless health coordinator, and a social service coordinator.

Services

ECHC provides comprehensive primary care, minor acute care, case management, basic laboratory and x-ray services, wellness education, immunizations, prescription assistance, dental care, optometry, emergency services, school physicals, Hoosier Healthwise enrollment, Indiana State Department of Health Breast and Cervical Cancer Prevention program, breast health education/screening, and transportation assistance. Specialized team members provide mental health/substance abuse services. In collaboration with the University of Southern Indiana, a registered nurse case manager coordinates primary care services for high risk inmates following release from jail. Health care services are provided at three fixed sites. ECHC's outreach teams provide services to 90 percent of Vanderburgh County homeless shelters. Volunteer physician specialists donate time and services to clinics.

Linkages

ECHC links with Southwestern Indian Mental Health Center for substance abuse, detox/treatment, and outpatient mental health. For shelter services, ECHC has linkages with Salvation Army Booth Manor, United Caring Shelters, YWCA, Albion Fellows Bacon Center (domestic violence), Goodwill Family Shelter, ECHO Housing Corporation,
Evansville Christian Life Center, Evansville Rescue Mission, House of Bread and Peace, Life Choice, and Ozanam Family Shelter. ECHC also has linkages with the Vanderburgh County Health Department, Council on Aging, Division of Family and Children Services, Community Action Program, Township Trustee, University of Southern Indiana School of Dental Hygiene, two local hospitals, and a network of area dentists and physician specialists. ECHC works closely with the Evansville Coalition for the Homeless.

Project Description

The Homeless Initiative Program (HIP) provides comprehensive primary health care services to men, women, and children who are homeless in Marion County.

Services

The Homeless Initiative Program is a multi-disciplinary team consisting of a family practice physician, a family nurse practitioner, two medical assistants, a pre-natal care coordinator, three outreach workers, five case managers, four employment specialists, and a program manager. Services are provided on the streets, at the HIP office, and in 15 shelter, mission, and transitional housing sites. Outreach staff assist homeless individuals with meeting basic needs for emergency shelter, food, clothing, health, and safety and provide referrals to HIP case management. Case managers assess the psychological and social needs of the individual, assist with the development of a care plan, and provide support, advocacy, and referrals as the individual acts on the plan. Employment and training services are available at the HIP office and include vocational counseling, pre-employment workshop, and assisted job search and job placement. Additional services for veterans seeking employment are available through the Homeless Veterans’ Reintegration Program at HIP. The mobile medical team provides acute and chronic care, primary care, free pregnancy testing, free vaccinations for children to protect them from serious illness, and referrals to specialists and hospitals, including referrals for dental care, foot care, and eye exams. A registered nurse facilitated access to pre-natal care, and provides education, support, and assistance to women until the baby is 6 weeks old. In a new initiative, the Homeless Initiative Program is a partner with Goodwill Industries, Easter Seals Crossroads, the Indianapolis Private Industry Council, and the City of Indianapolis in providing permanent supportive housing, employment, and resource coordination for 42 chronically homeless individuals.

Linkages

Patients who are homeless and in need of emergency, subspecialty, or hospital care are referred to Wishard Memorial Hospital or their Medicaid managed care provider. Linkages are in place with Salvation Army Harbor Light and Pathway to Recovery for residential substance abuse treatment and with Midtown Community Mental Health Center and Adult and Child Behavioral Healthcare for mental health evaluation and treatment. The medical team partners with 12 shelters, mission, and transitional housing sites where medical services are provided at least weekly. Other HIP staff coordinates services through more than 30 different agencies.

Project Description

Project Homecoming provides medical, dental, and social services to individuals who are homeless in St. Joseph County. These services are provided with an emphasis on preventive care and education with the goal of homeless patients maintaining a positive health status. Project Homecoming is located within a six-city block area on the south side of South Bend where both sheltered and unsheltered individuals dwell. All services and center hours are designed to best accommodate the needs of individuals experiencing homelessness.

Services

Medical services include adult, pediatric, and women's health; dental services; diagnostic lab tests; pharmaceuticals; prenatal care; maternal delivery; newborn hospital services; wellness care; acute care; Medicaid enrollment; prenatal classes; case management; smoking cessation classes; and social services.

Project Homecoming has linkages with Advocacy Center, Dismas House, Dolly's House, Center for the Homeless, Safe Station, Hope Rescue Mission, Life Treatment Center, Broadway Christian Parish, Portage Township Trustee, South Bend Housing Authority, YWCA, BABE Outlets, Big Brother/Big Sister, Boys and Girls Club, Casie Center, Catholic Charities, St. Vincent de Paul, Clay Township Trustee, Forever Learning Center, German Township Trustee, Hannah's House, Minority Health Coalition, Goodwill, and Community Resource Center.
