Project Description

Aunt Martha's Youth Service Center, through the Health and Outreach Center, provides comprehensive primary health care and ancillary services to persons who are homeless in Kane County. The Health and Outreach Center targets the cities of Aurora and Elgin. Aunt Martha's, along with its partners, uses a mobile health team to provide services at organizations serving homeless persons, as well as clinic-based services.

Services

Aunt Martha's Mobile Health Team provides shelter-based services at five local organizations in Aurora. These services are provided by a social worker, family practitioner, two medical assistants, and support staff. Additionally, Aunt Martha's provides primary health care, case management, and ancillary services at a clinic site devoted to serving individuals who are homeless in and around Aurora. Oral health care is provided by Community Health Partnership, with referral and linkage to area providers for substance abuse, mental health, and social services. Greater Elgin Family Care provides services through their Mobile Health Team for Elgin-area persons. Primary health care, oral health care, ancillary services, and case management are also provided by Greater Elgin Family Care Center at their Health Center. Referral and linkage to Elgin-based providers are made for substance abuse, mental health, and social services.

Aunt Martha's has linkages to Mutual Ground, Salvation Army, PADS/Hessed House, Community Counseling Center, Wayside Cross, Kane County Health Department, Open Door Clinic, and Rainbow Clinic at Hessed House.

Project Description

Central Counties Health Centers (CCHC) operates Capitol Community Health Center on the east side of Springfield. CCHC has been in operation since 1999 and began receiving HCH funding in the summer of 2003 at which time it began providing services at four community sites.

Services

HCH services are provided at the Salvation Army Citadel, Salvation Army Adult Rehab Center, Contact Ministries, and St. John's Breadline. CCHC contracts with Complete Care Pharmacy to administer the 340b drug pricing program and has staff dedicated to enrolling and monitoring patients in pharmaceutical patient assistance programs. CCHC collaborates with Mental Health Centers of Central Illinois for Projects for Assistance in Transition from Homelessness (PATH) which assists those with mental illness who are homeless.

Linkages

CCHC has linkages with more than 20 local social service agencies, many of which also participate in the local continuum of care.

Project Description

Crusaders Central Clinic Association’s Primary Health Care for the Homeless Program (PHCHP) provides regularly scheduled services at satellite clinics where persons who are homeless frequently congregate.

Services

Primary care services are delivered at several sites in the Rock River Valley area, including the Red Cross Shelter, Salvation Army Rehab Center, Hope Place, WAVE Domestic Violence Shelter, The Carpenter's Place, and several soup kitchens. A multi-disciplinary team of approximately 10 FTE staff help clients access medical, dental, mental health, substance abuse, housing, and entitlement resources. Crusaders Clinic has developed a chronic disease model of care for hypertension, diabetes, hepatitis C, asthma, and substance abuse. Nurses and case managers provide outreach services, identify clients, eliminate barriers to care, and monitor clients' progress.

Linkages

Crusaders Central Clinic has linkages with the local mental health agency, soup kitchens, women's shelters, Salvation Army Rehab Center, Red Cross Shelter, Hope Place, Carpenter's Place, youth shelters, and U.S. Department of Housing and Urban Development (HUD) transitional living sites.

Project Description

Heartland Health Outreach (HHO) is a wholly owned subsidiary of the Heartland Alliance for Human Needs and Human Rights with 18 years experience in the delivery of city-wide homeless health care services. In 1984, HHO received a Robert Wood Johnson Health Care for the Homeless Demonstration Program grant and since 1987, HHO has been the recipient of HCH program funding.

Services

HHO has developed a comprehensive system of health services, including mental health services (case management, residential, day programming, psychiatric, counseling), HIV testing and counseling, primary care, case management, housing, and dental services for persons with HIV. HHO's core model of service delivery integrates primary health care and social services in an outreach team effort. In addition to on-site delivery, services are provided in overnight and transitional shelters, drop-in centers, and meal program sites throughout Chicago. Four subcontracted health centers expand the same range of services to the south and west sides of Chicago, as well as Dupage and Lake Counties.

HHO has linkages with an array of providers for comprehensive health care, inpatient psychiatric care, substance abuse treatment, outpatient mental health, housing, and entitlement benefits.
