Project Description
Dirne Health Centers offers services to those experiencing homelessness in Kootenai County and the surrounding area through an integrated HCH/CHC program utilizing outreach & case management.
Services
Dirne provides continuity of care for uninsured, underinsured, and homeless patients through site-based clinics & in collaboration with area supportive service providers also serving homeless populations. Medical Services provided: Primary family medical care including: routine gynecological care, routine physicals, minor care for orthopedic injuries & lacerations, preventive care, immunizations, well child exams, chronic illness management (diabetes, asthma, depression, etc.), urgent care, and low cost, discount prescription coverage. Dental Care including comprehensive and emergency dental care to Medicaid and uninsured patients of all ages, including comprehensive exams, cleanings and fluoride treatments, sealants, and restorative care and referral for specialized care. Mental Health Treatment including marriage counseling and individual adult counseling.

Project Description
The Terry Reilly Health Services (TRHS) Health
Care for the Homeless Project serves persons
who are homeless in Ada and Canyon Counties.
Services
TRHS provides medical, dental, and social
services to individuals and families who are
homeless through site-based clinics and shelter-based
outreach clinics. The primary locations for
delivery of homeless services are the Boise
Clinic and the Nampa Clinic. TRHS also operates
shelter-based outreach clinics at seven homeless
shelters in the Boise/Nampa area. Services include
primary health care, diabetic education, pharmacy,
lab work, psychiatry, counseling, and case management.
Dental care is provided through the TRHS-Boise
Dental Clinic and the TRHSCanyon Dental Clinic.
Linkages
Shelter-based outreach clinics are operated
at the Women's and Children's Alliance, City
Light Women's Shelter, Valley Crisis Center,
Boise Rescue Mission, Nampa Lighthouse shelter,
Community House, and the Nampa Community Family
Shelter. Through two U.S. Department of Housing
and Urban Development (HUD) Supportive Housing
Program grants, TRHS and the Ada County Housing
Authority provide housing assistance and mental
health treatment to homeless patients with co-occurring
disorders. Substance abuse treatment, specialty
care, and other support services are performed
off-site through referral arrangements.
