Project Description
Community Health Care (CHC) HCH program provides
comprehensive primary health care to children
and adults who are homeless throughout the Quad
Cities area. Services are provided at six outreach
locations.
Services
The HCH outreach program provides preventive
and acute medical visits, medications, outreach,
social service coordination, and case management
to men, women, and children who are homeless.
Referrals are made for substance abuse treatment
and mental health services. The HCH program
also provides one emergency dental visit, dental
referrals, HIV counseling/testing, shelter,
and referrals for housing, job employment, food,
and clothing. The program is provided at six
sites–one on a full-time basis and five
visited weekly or bi-weekly.
Linkages
CHC's outreach program has extensive linkages
with shelters, transitional housing programs,
local pharmacies, food pantries, eye care providers,
and thrift stores. Referrals are made for mental
health services and substance abuse/addictions
treatment.

Project Description
People's Community Health Clinic (PCHC) has
a homeless health care team which provides outreach
services and refers persons who are homeless
to needed services.
Services
The homeless health care team visits shelters,
street locations, and transitional housing units
to assess persons who are homeless, to encourage
them and arrange for them to receive medical,
substance abuse, and mental health services.
PCHC's physicians, PAs, and nurse practitioners
provide preventive and acute health care services.
A certified substance abuse counselor is on
staff for evaluations of clients. The center
also has a dental clinic.
Linkages
People's Clinic is a member of the local Homeless
Coordinating Board. PCHC works cooperatively
with local antipoverty agencies, the food
bank, shelters, County relief offices, VA, and
other State and Federal agencies providing services
to persons who are homeless. Linkages are also
in place with faith based organizations who
assist individuals who are homeless with their
basic needs. PCHC has a referral relationship
with Black Hawk/Grundy Mental Health Center
for mental health services. Pathways and Covenant
Medical Center are local organizations that
provide chemical dependency treatment.

Project Description
The Outreach Project (TOP) of Primary Health
Care is a blend of a McKinney-funded project,
including three maternal/child-funded programs
serving pregnant and parenting families through
Healthy Opportunities for Parents to Experience
Success/Healthy Families America, Healthy Start,
and Empowerment. TOP also works closely with
Projects for Assistance in Transition from Homelessness
(PATH), CAP, U.S. Department of Housing and
Urban Development (HUD) and other community-based
mental health and housing programs. Last year,
TOP completed a food assistance campaign for
the State of Iowa through a grant from the U.S.
Department of Agriculture and the Food Assistance
Program. The Outreach Project maintains outreach
clinics in missions, shelters, and multi-service
agencies. Project staff conduct outreach visits
around or near bridges and in abandoned buildings
where individuals who are homeless congregate.
Services
The clinic operates weekdays providing services,
including primary health care, substance abuse
screening, mental health screening, referrals,
and advocacy. The program is staffed by approximately
1.8 FTE providers. Outreach staff travel daily
throughout the city. The two free-standing sites
are located at the House of Mercy and The Homes
of Oakridge campus. Services recently added
include chronically homeless-street outreach,
food assistance outreach enrollment, mental
health outreach services, and a Social Security
claims station.
Linkages
TOP works closely with other McKinney-funded
programs in Des Moines serving youth and young
mothers. TOP has established linkages with 100
agencies and organizations, including area shelters,
Iowa Department of Human Services, substance
abuse treatment centers, Des Moines Department
of Housing Assistance, and religious and private
organizations. Other community connections include
Healthy Start, Visiting Nurses Association,
and Iowa Empowerment.
