Project Description

St. Joseph's Mercy Care Services (SJMCS) has a multi-cultural focus, serving clients who are African-American, Hispanic, Asian, and Caucasian. Special emphasis is placed on meeting the primary medical needs of the homeless and immigrant populations in metropolitan Atlanta and providing the associated supportive services they may need.

Services

Services are offered at four fixed site clinics and at several mobile clinics. Delivery of services at mobile clinics is accomplished using a system of five vans and a mobile coach. Services at fixed sites are available 5 days a week, with evening clinics at certain locations and at mobile clinics according to a published monthly schedule. A combination of paid staff and volunteers provides services at approximately 40 locations each month. SJMCS provides primary health care, health education, immunizations, HIV counseling and testing, AIDS outreach, an infectious disease program, dental services, women’s health care, and supportive social services. Substance abuse residential recovery programs are provided in collaboration with several private non-profit organizations.

Linkages

Referrals and ancillary services are provided at Grady Memorial Hospital and other local hospitals and mental health centers. Saint Joseph’s Mercy Care Services has a close working relationship with Saint Joseph’s Hospital of Atlanta for referrals and has established numerous informal relationships with physicians who provide pro bono or low cost services.

Project Description

Union Mission, founded in 1936, works to prevent and end homelessness in Greater Savannah, Chatham County.

The J.C. Lewis Health Center, a 32-bed respite care facility for the homeless, opened in September of 1999. The Center also operates outlying clinics, including the Social Apostolate and Salvation Army. The Center provides primary holistic health care through an array of venues, including outpatient clinics, off site clinics at other agencies (Social Apostolate, Salvation Army), respite care program, and educational services. The State of Georgia has chosen J.C. Lewis Health Center as a model for providing health care to homeless individuals. The Center provides technical assistance to other agencies throughout the State of Georgia.

Services

The J.C. Lewis Health Center provides holistic health care in Savannah, Georgia. Services include primary healthcare, respite care, oral health care, eye clinics, behavioral health services, substance abuse counseling, health promotion and education, disease prevention and screenings, and complementary therapies such as acupuncture and massage.

Union Mission's partners include Memorial Health University Health Center, St. Josephs/Candler Health System, Chatham County Health Department, City of Savannah, United Way, Georgia Department of Community Affairs, U. S. Department of Housing and Urban Development, Georgia Region 13 for Substance Abuse, Mental Health and Mental Retardation, Recovery Place of Savannah, America's Second Harvest of the Coastal Empire, and The Robert Wood Johnson Foundation. The J.C. Lewis Health Center has formal protocols for referral and admission with Memorial Health University Medical Center and St. Joseph’s/Candler Health System. The J.C. Lewis Health Center also has linkages with the Savannah Health Mission and the Community Health Center.
