Project Description

The Henrietta Johnson Medical Center (HJMC) Homeless Health Care Access Program expands access to primary
health care for persons who are homeless in Wilmington.

The Homeless Health Care Access Program provides medical care and OB/GYN services to all individuals who are
homeless. Individuals needing assistance are identified by the Ministry of Caring through their outreach activities.
In addition, HJMC provides dental care for children. The Ministry of Caring provides outreach and case
management services to persons who require medical and dental services. The Ministry of Caring also refers all
adults requiring dental care to their own dental program.

HJMC maintains a specialist referral network located throughout the Wilmington area to serve all patients requiring
additional medical and dental treatment. Physicians at HJMC have staff and admitting privileges to all hospitals in
the area. Working relationships have been established with mental health and substance abuse programs, social
service organizations, community centers, churches, and civic organizations. The project coordinator at HJMC
works with Ministry of Caring case managers to refer individuals in need of such services to the appropriate
programs.
