Project Description
Through its Stout Street Clinic, Colorado Coalition
for the Homeless (CCH) has been providing integrated
health care, mental health care, dental services,
substance treatment services, and housing assistance
to people who are homeless in metro Denver since
1985. Since 1993, through Public Housing Primary
Care, health care and mental health care have
also been provided to residents of the Denver
public housing development, known as Quigg Newton,
and the immediately surrounding neighborhood.
Services
Services include preventive and primary care,
prenatal, pediatric, dental, ophthalmic/optometric,
mental health, substance abuse treatment, pharmaceutical,
outreach, case management, benefits acquisition,
a children’s center, medical respite,
vocational, residential, supportive housing,
rental assistance, and permanent affordable
housing. Colorado Coalition for the Homeless
also provides education and advocacy activities
throughout the State of Colorado, including
an annual conference focusing on homelessness.
CCH’s housing development activities have
created more than 1,000 units of housing for
people who are homeless or at risk of homelessness.
Linkages
Denver Health Medical Center (DHMC) provides
emergency, inpatient, and specialty services.
St. Joseph’s Hospital provides radiology
services. SSC maintains other linkages with
hospitals, educational institutions and homeless
service agencies. Other linkages include Metro
Denver Homeless Initiative (U.S. Department
of Housing and Urban Development (HUD) continuum
of Care), and Mental Health Center of Denver.

Project Description
Metro Community Provider Network (MCPN) Homeless
Project addresses the needs of people experiencing
homelessness in Aurora and Lakewood.
Services
Services include primary care medical services,
substance abuse and mental health counseling,
case management, outreach, human service referrals,
and other forms of assistance. Each clinic is
equipped with multiple exam rooms and is staffed
by a physician, nurse practitioner, and case
management/care coordinators/outreach services.
All clients have access to other services in
the larger MCPN system, including colposcopy,
ultrasound, physician support and consultation.
Through referral arrangements, clients have
access to mental health, substance abuse, dental
and pharmaceutical services.
Linkages
MCPN has numerous linkages in the community
to serve individuals who are homeless. Through
referral mechanisms and on-site access, services
are provided by Aurora Mental Health, Jefferson
Center for Mental Health, Aurora Housing Authority,
Comitis Crisis Center, Inter-Faith Ministries,
Tri-County Health Department, Jefferson County
Health Department, and the Colorado Coalition
for the Homeless.

Project Description
Peak Vista Community Health Centers, Inc.
(CHCI) provides primary health care to individuals
who are homeless. Services are provided at the
Homeless Health Center.
Services
A mid-level health professional provides services
weekdays at the Homeless Health Clinic located
five blocks from the shelter. In addition, a
physician provides part-time on-site services
and a female clinician provides a women’s
health clinic. Staff provides outreach services
at the local shelters and homeless camps. To
ensure continuity and quality of care, the provider
often takes his medical backpack and searches
the streets and camps to give patients their
test results and treatment plans. A medical
van provides a variety of services to individuals
living on the streets. Emergency dental services
and substance abuse counseling are also provided
on-site.
Linkages
Substance abuse treatment and counseling are
provided through arrangement with the health
department's McMaster's Clinic. Inpatient detoxification
and treatment are arranged by referral. Patients
with dental needs beyond the scope of the Homeless
Health Center are referred to Peak Vista CHC’s
main dental clinic. The Homeless Health Center
also has linkages with Marion House and Soup
Kitchen social outreach team, Catholic Charities,
Urban Peak, El Paso County Department of Health
and Environment.

Project Description
Pueblo Community Health Center (PCHC) provides
primary health care services in one location
at Posada (homeless support center) in downtown
Pueblo where the majority of individuals who
are homeless dwell.
Services
PCHC provides a mid-level provider, nurse,
and project coordinator for primary health care,
substance abuse evaluation, and referral assistance.
PCHC provides on-site substance abuse evaluations,
counseling, and referrals to Crossroads Managed
Care, a substance abuse treatment provider.
The center also provides educational information,
both verbal and written, so patients will be
aware of options for healthier lifestyles. The
project coordinator maintains and evaluates
day-to-day policies and procedures. Referrals
to 24-hour emergency services, radiology services,
mental health counseling, dental services, and
pharmaceutical services are also included in
this centralized project. Posada provides building
space and the on-site services of an outreach
case manager and transportation specialist.
The case manager’s primary responsibilities
include coordinating community support services
to help individuals obtain safe, affordable,
and permanent housing. Specific responsibilities
include outreach and addressing an individual’s
immediate survival needs. The transportation
specialist transports individuals and families
to referral sites. Cab and bus vouchers are
also provided.
Linkages
Spanish Peaks Mental Health Center provides
mental health case management services for adults
and children through the Federal Project for
Assistance in Transition from Homelessness (PATH).
PATH employees refer individuals who are homeless
to substance abuse treatment programs provided
through local organizations, as well as provide
emergency mental health evaluations and interventions,
psychiatric evaluation and treatment services,
consumer advocate services, and residential
services. Crossroads Managed Care accepts referrals
for substance abuse treatment. Other informal
linkages include the Pueblo County Department
of Social Services, providing entitlement and
eligibility assistance, as well as the YWCA,
a domestic violence shelter which schedules
informational sessions about homeless-related
issues for their clients. Services are also
provided through Posada's central service center
