Project Description
The Alameda County HCH Program (ACHCHP) is
located in the Alameda County Health Care Services
Agency and provides comprehensive health and
support services to people who are homeless
county-wide.
Services
Services include mobile primary care at 27
shelters and 2 alcohol/drug recovery centers,
mobile oral health and subcontracted care, primary
care subcontracts at 10 community clinics and
Alameda County Medical Center, and an array
of critical support services, including case
management, outreach, referral, respite care,
benefits counseling, health education, HIV/AIDS
counseling, mental health linkages, transportation,
and assistance for housing, employment, food,
and nutrition. The program conducts bi-monthly
regional workshops for health and social service
providers, homeless coalition members, and others.
Linkages
Linkages include social service, government,
organizations, advocacy, and referral to mental
health services and emergency shelters. The
program implements a U.S. Department of Housing
and Urban Development (HUD) supported Homeless
Families Program, demonstrating the efficacy
of coupling housing with health and support
services. In 1991, the ACHCHP was honored by
the St. Joseph Health System with the Intensive
Care for Our Neighbor (ICON) Award for excellence
and innovation in provision of health care to
indigent people. ACHCHP participates in various
emergency services networks. In 1995, the ACHCHP
received the California Public Health Association/North
award for outstanding community service. The
program also participates in the Alameda County
Continuum of Care Council.

Project Description
CARE LINK - Community Medical Centers provides
comprehensive primary health services at area
shelters, halfway houses, transitional/treatment
programs and on the street through its medical
outreach program. Follow-up primary care is
available through its Channel Medical Center
and dental services at San Joaquin Valley Dental
Group in Stockton.
Services
The Channel Medical Clinic is located in a
medically underserved area and serves patients
from homeless shelters in the downtown and mid-town
sections of Stockton and the surrounding San
Joaquin County areas. Services include primary
medical care for acute/episodic and chronic
care, well child exams and pediatrics, women’s
health, perinatal and GYN, laboratory, x-ray,
pharmacy, nutrition, dental, medical outreach
and case management. A total of 260 confidential
HIV (OraSure) tests were provided through the
outreach program in addition to clinic sites
and the program provided more than 500 immunizations
for Hepatitis A & B, Tetanus and Influenza
to homeless patients this past year. Medical
outreach services are provided through weekly
visits to area shelters by an integrated medical
and street outreach services team.
Linkages
Linkages with substance abuse and mental health
services are available from San Joaquin County
Health Care Services and H.E.A.R.T. (S.J. County
Mental Health’s homeless outreach program).
The Council for the Spanish Speaking, County
Human Services Agency, and San Joaquin AIDS
Foundation ensure coordination of services and
support for clients. CARE LINK coordinates the
Stockton Community Coalition for the Homeless
with more than 30 agencies actively involved
and a Homeless Task Force that brings together
agencies and policymakers in city and county
government to address issues of homelessness.
Linkages for housing services also include Central
Valley Low Income Housing and San Joaquin County
Housing Authority along with emergency shelter
providers. CARE LINK -- Community Medical Centers
has collaborated with San Joaquin County Housing
Authority and Stockton Shelter for the Homeless
to develop a Misión Campesina Project
that provides shelter, employment, and health
care services to unaccompanied migrant workers
who have traditionally relied on shelter services
or lived on the streets during the extended
Central Valley growing seasons.

Project Description
The Center for the Vulnerable Child (CVC)
at Children's Hospital and Research Center provides
comprehensive care and a medical home for children
who are homeless or at risk for homelessness,
including children who live in biological or
foster families and children who have been newly
reunified with their parents. This is done by
addressing health, psychological, and social
problems that frequently accompany poverty.
The service area is Alameda County.
Services
The Center for the Vulnerable Child is a department
with clinical services, research, and policy
divisions at a nonprofit children’s hospital.
As a prevention and early intervention program
that features coordination of services through
case management, CVC provides both a medical
home and a one-stop-shopping model of comprehensive,
interdisciplinary services for high risk children
and families. Services include comprehensive
medical, mental health, developmental, family,
and social service assessments, specialist and
emergency health care, individual and family
counseling, infant/parent psychotherapy, family
support groups, therapeutic nursery services,
and outreach services.

Project Description
Clinica de Salud Del Valle de Salinas (CSVS)
is one of the few providers in Monterey County
for individuals who are medically indigent or
homeless. Under the umbrella of CSVS, individuals
receive comprehensive care, including basic
health screenings, immunizations, basic oral
screenings, orthopedics, and podiatry.
Services
The program is implemented at all Clinica de
Salud locations. In addition, care is delivered
via a mobile clinic which regularly visits homeless
shelters and encampments. The mobile clinic
serves as a key access point for those seeking
primary care, oral health, and mental health
and substance abuse treatments. Specialty services
are referred to Natividad Medical Center's main
campus.

Project Description
Clinica Sierra Vista's Homeless Mobile and
Respite Services is a community health center
program, serving clients throughout the 8,000
square miles of Kern County in California's
Central San Joaquin Valley. The program operates
a clinic in the city of Bakersfield and a 42-foot
mobile unit that travels to sites throughout
the county. The program also includes a 12-bed
medical respite unit.
Services
Primary and preventive health care and social
services are provided on-site. Dental, laboratory,
and radiology services are available at other
Clinica facilities. Four full-time employees
are supported by a cadre of volunteers, including
physicians from a local teaching hospital, who
provide medical care at either the homeless
clinic or at regularly scheduled mobile unit
sites, such as emergency shelters, low income
motels, farm labor camps, schools located in
economically depressed neighborhoods, and homeless
encampments. The mobile unit is also used for
HIV and tuberculosis screening programs, immunization
clinics, WIC enrollment, and community events
such as fairs and festivals promoting healthy
lifestyles through health education. Clinica's
respite project provides short term care to
homeless adults who are too ill to manage on
the streets or in shelters. The program, located
adjacent to the homeless clinic, operates at
its capacity of 12 clients who receive medical,
nursing, and social service assistance with
benefits application, housing placement, enrollment
in addiction and mental health treatment programs,
and other ongoing assistance.

Project Description
Community Health Centers of the Central Coast
is a network of 16 licensed primary medical
and dental care clinics that provide services
to the homeless throughout San Luis Obispo County
and northern Santa Barbara County. Services
for the homeless are provided at all clinics
and through the use of four mobile medical/dental
vans. CHC networks with many community agencies
that serve the homeless to fill gaps in the
continuum of care. This involves active participation
in the Homeless Services Coordinating Committee,
Adult Services Policy Council, SLO Hepatitis
C Coalition, AIDS Support Network and SL County
HIV Prevention, Advocacy, and Care Consortium,
SLO County Medical Society, and others.
Services
Community Health Centers of the Central Coast’s
HCH Program provides urgent health care services,
including health screening and assessment, dental
screening, health education, on-site medical
treatment and/or referral, and transportation
to medical treatment. The program also provides
mental health counseling and substance abuse
counseling referrals. Program staff provides
case management so individuals can receive organized
and improved access to medical, dental, mental
health and substance abuse treatment services,
entitlement benefits, and social service benefits.

Project Description
The HCH Project has been operated by Contra
Costa Health Services (CCHS) since 1990 and
is an essential component of the primary Contra
Costa Health Services delivery system. The project
provides health care services through mobile
clinics and through the CCHS integrated ambulatory
system of care and the CCHS Departments of Mental
Health and Alcohol and Other Drugs. Mobile clinic
services are provided via two part-time large
mobile clinics and a full-time pharmaceutical
mini-bus. The mobile team comprises a medical
director, three part-time family nurse practitioners,
two part-time public health nurses, a community
health worker, financial counselor, mental health
specialist, substance abuse counselor and administrative
assistant. All team members, under the supervision
of the project coordinator, carry out the support
services component of the HCH Team.
Homeless patients who receive care on one
of the mobile clinics are referred into 10 CCHS
ambulatory care health centers for primary and
specialty care, into County mental health and
substance abuse services and to Contra Costa
Regional Medical Center for emergency, inpatient
hospital, outpatient surgeries, laboratory,
and radiology. There are currently three ambulatory
care clinics, located throughout the County,
that are designated specifically for homeless
patients to help them transition from the mobile
clinic setting into the ambulatory care system.

clinics
Services
Primary health care services provided by the
HCH Mobile Team include routine physical assessments,
basic treatment of primary health problems,
such as minor wounds and skin conditions, respiratory
problems, TB, and acute communicable diseases.
Referrals are made for follow up treatment of
identified health care needs. Substance abuse
and mental health services are also provided.
The HCH pharmaceutical mini-bus has medical
supplies, equipment, and medications. The CCHS
health care delivery system provides primary
and specialty care including emergency department
services, inpatient hospital care, outpatient
surgeries, mental health and substance abuse
services, laboratory, and radiology.
Linkages
Clients referred for follow-up care or specialty
services, such as dental care, dermatology,
and prenatal care, are referred to the appropriate
division within CCHS for continued care after
first being attended to by the HCH Team.

Project Description
Family Health Centers of San Diego (FHCSD)
is the lead agency for the San Diego Health
Care for the Homeless Project. This HCH Project
serves the entire county of San Diego through
a network of community health clinics, social
service agencies, and homeless providers.
Services
FHCSD offers comprehensive services, including
adult and pediatric medicine, dental, optometry,
speech and hearing, laboratory, pharmacy, women's
health, and prenatal services. The project uses
an integrated team approach and case management
model, initiated through frequent visits into
the neighborhood and clinic sites. Project staff
include health care providers, case managers,
outreach workers, and dual diagnosis counselors
who provide substance abuse/mental health screening,
crisis intervention, education, and referrals.
Other services include translation assistance,
client advocacy, basic needs, shelter, and housing
referrals. The project also provides primary
care and emergency medical services at 19 community
clinic sites. A dental restorative project complements
basic dental care. New Access Point funding
has facilitated the creation of a small primary
care clinic with associated support services
at a veterans-related residential recovery facility.

The HCH Project has linkages with many social
service agencies and homeless service providers
as well as networking and referral support of
more than 50 agencies countywide.

Project Description
Gardner Family Health Network's Healthcare
for the Homeless Project (GFHN-HHP) provides
comprehensive health care to people who are
homeless at various shelters, drop-in centers,
and churches throughout Santa Clara County,
via mobile medical van and on-site clinics.
Services include acute medical care, social
and medical case management, health education,
substance abuse services, and referrals.
Services
GFHN-HHP provides a full range of services
to its clients. Primary health care services
include routine physical assessments, basic
treatment of primary health problems, respiratory
problems, women's health issues, TB, acute communicable
diseases, and STD and HIV testing. Men, women,
and children receive culturally sensitive health
care services, social and medical case management,
health education, and substance abuse services
at specified rotating locations in Santa Clara
County. Continuity of care is addressed by maintaining,
generating, and distributing to the community,
a monthly service calendar. GFHN-HHP collaborates
with the homeless service community to strengthen
the quality and scope of services. Other services
include translation assistance, client advocacy,
and referrals for basic needs, shelter, and
housing assistance. Clients needing services
beyond the scope of GFHN-HHP are referred to
other community agencies.

Loaves and Fishes, San Jose Family Shelter,
Bill Wilson Center (2 sites), Calvary Church,

Basilica of St. Joseph’s. Additional services
are provided at the cold weather shelter in
Gilroy established in the National Guard Armory
on a seasonal basis.

Golden Valley Health Centers Corner of Hope
Program exists to increase the health status
and health outcomes for individuals and families
who are homeless by increasing access to comprehensive
primary health care, substance abuse programs,
dental services, and mental health services.
The program strives to fill gaps in homeless
services in the rural county of Stanislaus.
The program stresses wellness and health promotion
for individuals who are homeless.
Services
Services are provided 5 days a week. Primary
care includes episodic, chronic care, well child
exams, women's health, perinatal and GYN care,
physical exams, diagnostic services, pharmaceuticals,
dental care, vision referrals, health education,
some mental health services, and supportive
services for substance abuse issues. Patients
are offered referrals and case management services.
Hospital and after hours care are available
through Golden Valley's on-call physicians.
Volunteers are welcome to assist paid staff.

Project Description
Mendocino Community Health Clinic (MCHC) has
been providing comprehensive primary health
services in rural northern California since
1992. The HCH program adds to MCHC’s family
of clinics in Ukiah, Lakeport, and Willits.
MCHC is the largest safety net provider for
ambulatory care in Mendocino and Lake Counties.
Services
MCHC provides services to individuals who are
homeless in the Ukiah area of rural Mendocino
County. Services are provided at the Hillside
Health Center site. Transportation is provided
by a Care-a-Van which has an established route
to the area homeless service agencies, including
the Buddy Eller Homeless Shelter, Ukiah Community
Center, Plow Shares Community Dining Room, Social
Services, and A Healing Cooperative (faith-based
mental health recovery support organization).
Street outreach is provided through a subcontract
with A Healing Cooperative. MCHC’s program
is the result of long-term collaboration of
health practitioners, advocates, and governmental
and service agencies and has grown to provide
services to 577 homeless people. Clinical focus
areas include primary, preventive, and chronic
medical care, behavioral health for patients
with dual diagnosis, and oral health assessments,
sealants, and services.
Linkages
MCHC has linkages with Ford Street Project
and Buddy Eller Emergency Shelter. MCHC is a
member of local collaboratives working with
the Mendocino County Department of Social Services
and Ukiah Community Center. As a member of Mendocino
County Homeless Services Planning Group, MCHC
has been in the forefront of developing a Continuum
of Care Plan to End Homelessness.

Mobile Health Care for Rural Homelessness in
Humboldt County is a project operated by Mobile
Medical Office, a community health center on
wheels. Mobile Medical Office delivers primary
care by taking the mobile clinic to several
isolated rural communities in Humboldt County
and to local soup kitchens.
Services
Mobile Medical Office provides medical and
mental health care, substance abuse treatment,
and case management services via a mobile unit
which visits shelters, soup kitchens, and neighborhoods
where people who are homeless congregate. A
team, comprising a registered nurse, substance
abuse counselor, social worker, and case manager,
provides outreach to transitional houses, homeless
encampments, food banks, and persons living
on the street. Dental services are provided
via contract with a dental clinic near the soup
kitchen which is located near the mobile unit.
Linkages
Mobile Medical Office works closely with St.
Vincent De Paul Society, the operator of the
soup kitchen visited by the mobile clinic. The
clinic has ongoing relationships with many providers
of human services in Humboldt County, including
domestic violence shelters, emergency and transitional
housing shelters, alcohol and drug care facilities,
and Humboldt County Department of Health and
Human Services. Mobile Medical Office is an
active member and participant in the Humboldt
Homeless and Housing Coalition, the Continuum
of Care organization for Humboldt County.

The Sacramento County Health Care for the Homeless
Program is part of the Clinic Services Branch
of the Primary Health Division in the Department
of Health and Human Services.
Services
The HCH Program collaborates with nonprofit
health services and resource programs, including
Catholic Healthcare West (Mercy), Loaves and
Fishes, University of California Davis Medical
School, many homeless shelter and program sites,
and eight county clinic sites, including a dental
clinic. The clinic sites provide primary and
public health services, health education, and
specialty referrals. Primary care includes acute
care, chronic disease management, assessment
and treatment, and health education. Referrals
are made for dental, vision, mental health,
alcohol and drug treatments, nutrition program,
and chronic disease programs geared towards
diabetes management and asthma. Secondary and
tertiary care is provided through contracts
with local not for profit hospitals. Outreach
services are provided by public health nurses
(1.5 FTE) and a registered nurse (1.0 FTE).
The PHNs coordinate two Well Child clinic sites
with the assistance of UCD third year pediatric
residents and a family nurse practitioner. These
clinics provide CHDP education workshops and
triage assessment. The outreach nurse provides
TB screening and testing. Clients are issued
TB clearance photo ID cards that are valid for
six months.

Project Description
The Santa Clara Valley Health and Hospital
System (SCVHHS) comprises five departments:
Valley Medical Center, Ambulatory and Community
Health Services, Mental Health, Alcohol/Drug
Services, and Public Health. The Valley Healthcare
for the Homeless Program (VHHP) resides in Ambulatory
and Community Health Services (ACHS), the primary
provider of outpatient services. ACHS operates
a network of 10 fixed sites, along with a shelter
clinic, two mobile medical units, and two mobile
dental units. Nine of ACHS’s fixed sites,
along with a mobile medical unit and a mobile
dental unit form 11 access points for health
care to individuals who are homeless or near-homeless
and other vulnerable groups.
Services
ACHS provides a full range of primary and specialty
care, dental, and ancillary services, including
laboratory, radiology, pharmacy, health education,
care management, nutrition, and social services.
The service strategy is to use a care coordination
model to create a "medical home" to
improve access to health care, enhance health
services, promote continuity of care, and strengthen
linkages with non-medical housing and support
programs.
Linkages
ACHS works in partnership with its sister
agencies in SCVHHS, other county departments,
Emergency Housing Consortium, Gardner Family
Health Network (also an HCH grantee), Community
Health Partnership, a consortium of community
clinics, Children's Health Initiative, an insurance
outreach effort, and an array of shelter, housing,
food, job, faith-based, and other support services
that serve individuals who are homeless. Additionally,
ACHS collaborates with the Santa Clara County
Collaborative on Housing and Homeless Issues
on health care, housing, basic needs, and other
services for homeless individuals.

Project Description
The Santa Cruz Homeless Persons Health Project
(HPHP) provides comprehensive health and support
services in both urban and rural settings in
Santa Cruz County.
Services
HPHP provides comprehensive care and support
services through a small primary care clinic
and dispensary, two county clinics, four multidisciplinary
service teams, and a network of community programs.
The Adult and Youth/Family services teams operate
a "black bag" public health nurse-anchored
model, providing outreach, a drop-in nurse clinic,
harm reduction, referrals, social services,
HIV counseling/testing, and transportation assistance.
HPHP collaborates with county mental health
and substance abuse services to implement integrated
services emphasizing Housing First and employment
for individuals with serious mental illness.
Linkages
In Santa Cruz, linkages include Rebele Family
Shelter, Page Smith Community House transitional
housing program, Inter-faith Satellite Shelter
Program, Homeless Community Resources Center
(multi-service day center). Additional linkages
include agencies participating in a U.S. Department
of Housing and Urban Development (HUD)-linked
Continuum of Care planning body, River Street
Shelter, Santa Cruz Community Counseling Center,
Santa Cruz AIDS Project Drop-In Center, Sienna
House Maternity Home, Mental Health Client Action
Network, County Mental Health and Substance
Abuse Services, and Dominican Hospital. In Watsonville,
linkages also include Salvation Army, Loaves
and Fishes, Pajaro Valley Shelter Services,
and Watsonville Community Hospital.
