Project Description

Birmingham Health Care (BHC) provides comprehensive urgent, acute, and primary health care services with linkages to community providers when needed. BHC is accredited by JCAHCO for both its ambulatory and behavioral health care services and is licensed by the Alabama Department of Mental Health and Mental Retardation.

Services

Medical services include urgent and primary care, treatment of acute and chronic illness, physical exams, family planning, diabetes testing and care, immunizations, flu vaccinations, x-rays, blood pressure monitoring, health education, nutrition counseling, well child check-ups, HIV and TB screening and education, optometry services, treatment for Black Lung, asthma, chronic bronchitis and emphysema, and referrals for specialty care. Dental services include emergency, primary, and preventive dental services for children and adults, treatment and x-rays, replacement of missing teeth, crowns, bridges, and partials. Behavioral health services include case management, intensive outpatient substance abuse treatment, mental health counseling, on-the-job job training, outreach, transportation, and child development services.

Linkages

Linkages include the county indigent care hospital, public health department, Mental Health Authority, local homeless shelters, and inpatient substance abuse treatment facilities. BHC works closely with local businesses, universities, specialty clinics, churches, and other faith-based groups to provide seamless health care. BHC played an integral role in establishing a system to connect homeless services in Birmingham, known as Metropolitan Birmingham Services for the Homeless (MBSH).

Project Description

Franklin Primary Health Center operates the H.E. Savage Health Care for the Homeless Program, which provides multidisciplinary services to individuals who are homeless or are at risk of being homeless, in a downtown urban “store front” clinic. These services are provided through linkages with seven community health centers and other agencies which also provide services to persons who are homeless.

Services

Services include primary health care, pediatrics, OB/GYN, referrals to specialty clinics, optometry, dental, x-ray, labs, and a 24-hour emergency on-call provider. Medications are provided through Franklin’s Pharmacy, Catholic Social Services, and prescription assistance programs. Outreach medical services are provided on-site at 15 Place (multi-service center) and The Breakfast Club (soup kitchen) located in a downtown church. In addition, behavioral health specialists, medical social workers, outreach case managers, and support staff are available to provide comprehensive services. Substance abuse assessment and referral services, as well as mental health services, are provided by licensed and credentialed professionals. Social services also include food, medications, assistance with housing, and additional referrals to community agencies. HCH also provides a dedicated van service.

Linkages

Mental health services are provided on-site and by referral to the Mobile Mental Health Center. Residential substance abuse treatment is provided by referral to the Waterfront Rescue Mission, Salvation Army, Wings of Life, Serenity Care, Home of Grace for Women, Peachford Program, and Franklin Primary Health Center’s Substance Abuse Program. Other linkages include Vocational Rehab Services, Goodwill Services, Veterans Administration, Social Security, and 15 Place Homeless Multi-Service Center. Transitional housing is provided through Catholic Social Services, Sybil Smith Village, Mobile Mental Health, and Volunteers of America Homeless Program.

Whatley Health Services provides comprehensive > primary health care to underserved persons including the uninsured and > homeless in Tuscaloosa, Hale, Bibb, Sumter, Lamar, Walker and Greene > counties, Alabama.

Services

Whatley Health Services, Inc. (WHSI) serves as part of a major network of comprehensive care within the West Alabama region. WHSI concentrates on the delivery of comprehensive care through the Maude I. Whatley Health Center and 12 additional clinic sites throughout the service area. In addition to the 12 centers, WHSI provides health care services to individuals who are homeless and at risk children by way of a Mobile Medical Unit and a free standing clinic in Tuscaloosa, Al. WHSI also provides medical services for inmates at the Tuscaloosa County Jail. The medical staff provides family practice, pediatrics, gynecology,and internal medicine. WHSI services are complimented by providing speciality services such as pre-natal, ear, nose, throat, mental health/behavioral health, case management and substance abuse. WHSI also provides comprehensive dental care which includes preventative, restorative, dentures, root canals, fillings,crowns, oral biopsies and bridges. A mobile health unit with medical and dental capabilities provides services to homeless users in outlying areas.

Linkages

Partnerships include the Salvation Army and Community Service Programs of West Alabama, a Community Action Agency. Whatley has linkages with the Alabama Department of Public Health, Alabama Department of Human Resources, Alabama Department of Vocational and Rehabilitation Services, West Alabama AIDS Outreach, Hale County Board of Education, Greene County Board of Education, Tuscaloosa County and City School Systems, Stillman College, Shelton State Community College, University of Alabama, United Way of West Alabama, United Way of Central Alabama, American Red Cross, DCH Regional Medical Center, Hale County Hospital, Greene County Hospital, Indian Rivers Community Mental Health Center, Phoenix house, Hannah Home and Tuscaloosa County Jail.
