Project Description

The Anchorage Neighborhood Health Center's (ANHC) mission is to provide high quality medical, dental, and health-related services, serving the needs of all members of the community, regardless of socio-economic status.

Services

Nurse practitioners, physician assistants, physicians, behavioral health specialists, dentists, nurses, and case managers provide primary, dental, and behavioral health care at two ANHC sites. In addition, they staff regular clinics at shelter sites, perform outreach, provide case management and health education, and coordinate referrals. A health care team holds clinic sessions at shelters, a rehabilitation center, and the local community mental health center. Routine services include well child, preventive care, immunizations, acute illness care, dental care, substance abuse screening, brief behavioral health assessments, and referral for substance abuse and mental illness. Patients have 24-hour access to care via the on-call physician staff.

Linkages

The project collaborates with several shelters: Brother Francis Shelter and Anchorage Rescue Mission (single adults); Clare House and Abused Women's Aid in Crisis Shelter (women and children); McKinnell Shelter (men and children or families); Adult Rehabilitation Program (adult males with chemical dependency); Anchorage Community Mental Health Center (single adults with mental illness); and Salvation Army Cares for Kids Program (crisis care for children in State protective custody). Additional substance abuse treatment is provided by Clitheroe Center. Following treatment, individuals are referred to a transitional care unit which serves as a halfway house. Following transitional care, individuals may reside at one of a few options for transitional, low-cost, alcohol and drug-free living. Mental illness treatment is referred to the Anchorage Community Mental Health Center, offering sliding fee services. Specialty dental services are referred to local area dentists.

Project Description

SouthEast Alaska Regional Health Consortium (SEARHC, pronounced "search") is a consortium of 18 Native communities in Southeast Alaska. It is one of the oldest and largest Native-run health organizations in the Nation, incorporated in 1975. SEARHC has succeeded in building a network of health care services, including prevention and education programs, outpatient clinics, and inpatient hospital services. This network now includes the Front Street Clinic, offering services to individuals who are homeless in Southeast Alaska.

Services

SEARHC provides health care services to individuals who are homeless at a facility in downtown Juneau. The clinic provides acute primary health care, dental services, case management, and referrals for substance abuse and mental health services. A family nurse practitioner (FNP), dentists, and dental hygienists provide services in the clinic. The FNP and case manager also provide services in outreach clinics.

The Front Street Clinic has linkages with many agencies in Juneau, including Juneau Alliance for Mental Health, Gastinuea Human Services, St. Vincent De Paul: Transitional Housing, Catholic Senior Services, Catholic Services, Glory Hole shelter and soup kitchen, Dept. of Public Health, Tlingit and Haida Central Council, AWARE, Juneau Recovery Center, WIC, and Salvation Army.
