Documenting Disability Training for Medical Providers

Purpose This training offers a special opportunity for clinicians to learn how they can expedite Federal disability benefits for individuals with severe medical impairments who are eligible for Supplemental Security Income (SSI) or Social Security Disability Insurance (SSDI). Participants will also learn why it is especially important for clinicians to support homeless individuals’ disability claims and how they can do so efficiently and effectively.

Target Audience The training is specifically designed for physicians, psychologists, and others who are authorized by the Social Security Administration to provide medical evidence of impairment in support of SSI/SSDI applications. Other medical providers (nurse practitioners, physician assistants, clinical nurse specialists, nurses) who assist with documentation of medical impairments are also encouraged to attend. Continuing education credit is provided through the American Academy of Family Physicians and an affiliate of the American Nurses Association.

Curriculum The training curriculum is based on this publication by the National Health Care for the Homeless Council:
Documenting Disability: Simple Strategies for Medical Providers (2007)
The training is 3 hours long. It focuses on what medical providers need to know and do to support applications for SSI/SSDI benefits and gives them an opportunity to talk about any problems they have encountered with a knowledgeable HCH provider, a local Legal Services attorney, and (optimally) representatives of the state Disability Determination Services (DDS) and local SSA field office.

Learning Objectives After attending this training, participants should be able to:
  • Explain why it is important for medical providers to play an active role in supporting applications for federal disability assistance (SSI/SSDI), especially those submitted by individuals who are homeless.
  • Specify disability criteria used by the Social Security Administration in determining eligibility for SSI and SSDI, and describe the disability determination process.
  • Use SSA's Medical Listing of Impairments to expedite disability benefits for persons with severe impairments.
  • Write a letter to the state Disability Determination Services in support of a SSI/SSDI claim, appropriately specifying the claimant's medical impairments and resulting functional limitations.
