The HCH Directory is intended to foster collaboration and communication among federally-funded Health Care for the Homeless projects, and to provide a starting point to find health care resources for homeless people.
The following listings were updated in the fall of 2009, when each grantee was asked to provide updated information; some grantees did not provide updates, but the most recent information available to us is posted here.
