CME/CE INFORMATION
Please register your attendance at plenary and workshop
sessions by having your name badge scanned by the
volunteer at the door. For the two morning plenary
sessions, register before 8:45 am, as you enter. For
workshop sessions, register as you leave. Shortly after each
session, you will receive an e-mail inviting you to evaluate
the session by simply clicking on a link in the e-mail. If you
seek Continuing Education credits, this process is required;
if not, please help us strengthen our conference by using
the process to provide your candid evaluation.
ACCREDITATION POLICIES
This conference is designed by HCH professionals for
HCH professionals, including clinicians and their support
staff; administrators; board members; and consumers of
HCH services. It will also be of value to others who
provide health care and support services to homeless
people, as well as government officials and advocates.
The desired result for this conference is an increased
understanding of the health care needs of homeless
people and the health care services that should be
provided to them.
At the conclusion of this activity, participants should be
able to describe and discuss:
- Unique elements of the HCH approach to patient care.
- The experiences of consumers of HCH services,
clinicians serving homeless people, and advocates for
homeless people throughout the nation, as they work
to maintain clinical quality, consumer participation,
and interdisciplinary approaches in clinic care.
- The systemic realities of barriers to care and methods to
remove those barriers at clinic and community levels.
This activity has been planned and implemented in
accordance with the Essentials Areas and Policies of
the Accreditation Council for CME (ACCME) through
the joint Sponsorship of Vanderbilt School of Medicine
and The National Health Care for the Homeless Council.
Vanderbilt School of Medicine is accredited by the
Accreditation Council for CME to provide continuing
medical education for physicians.
Vanderbilt School of Medicine designates this
educational activity for a maximum of 20.5 AMA PRA
Category 1 Credits™. Physicians should only claim
credit commensurate with the extent of their
participation in the activity. It is the policy of
Vanderbilt School of Medicine not to discriminate

It is the policy of Vanderbilt School of Medicine that
participants in CME activities be made aware of any
affiliation or financial interest that may affect the
speaker’s presentation(s). Each course director,
planner, and speaker has completed and signed a
conflict of interest statement. The faculty members’
relationships will be disclosed to the audience.
All course directors, planners, and speakers indicated
no financial relationships to disclose. This CME activity
received no commercial support.
Requirements for Successful Completion
Attendees wishing to receive credit for workshops
must attend the duration of the workshop, have their
nametag barcode scanned by the workshop room
monitor, and complete an online evaluation form for the
workshop.
Conflicts of Interest
The ANCC has established guidelines whereby all
speakers must disclose any affiliations which may
cause a conflict of interest.
A Conflict of Interest occurs when an individual has an
opportunity to affect educational content about health
care products or services of a commercial interest with
which she/he has a financial relationship. The following
conference presenters of this CNE activity listed below
have declared relevant financial relationships as
follows:

Non-Endorsement of Products
Approval of this continuing nursing activity refers only
to education activities and does not imply that there is
real or implied endorsement by the Tennessee Nurses
Association or the American Nurses Credentialing
Center’s Commission on Accreditation of any product,
service, or company referred to in this activity nor of
any company subsidizing costs related to the activity.
Off-label Product Use
Attendees will be notified when a presentation relates
to any product used for a purpose other than that for
which it was approved by the U.S. Food and Drug
Administration.
Attendees who have completed the required online
workshop evaluations will receive their CME/CE
certificates via email after the July 15 deadline.
This activity is currently in review for CE credit through the
National Association of Social Workers.
The California Certification Board of Alcohol and Drug
Counselors has awarded NHCHC a provider number and is
currently reviewing the activity application.
Applicants are responsible for checking with certifying
authorities in their states in advance of the conference regarding
CME/CE reciprocity and for complying with attendance and
