On-line registration for the National HCH Conference & Policy Symposium is now closed. You may register for the main conference, with payment (Visa, Mastercard, check or cash), on site in Phoenix. Registration for the June 11 Pre-Conference Institutes is closed altogether and on site registration will NOT be available for the Institutes.
Conference Registration covers all sessions (except Wednesday's full-day pre-conference institutes), materials, CME/CEU certificates, welcome reception, breakfast and lunch on Thursday and Friday, and breakfast on Saturday. Conference fees will be refunded only if cancellation is requested in writing by May 28.
Consumer Scholarships:
A limited number of stipends are available to support travel, food, lodging, and conference registration costs for homeless or formerly homeless members of HCH boards or advisory committees or for consumers who are staff members of HCH projects. Accommodations:
The conference will be held at the Hyatt Regency Phoenix. Rooms are available at the Hyatt Regency Phoenix at a rate of $199-$204 each night. A 12.27% tax will be added to the rate.
A limited number of rooms are available at the Wyndham Phoenix (located within walking distance of the Hyatt Regency Phoenix) at a special conference rate of $105 for a single/double and $125 for a triple/quad. Travel:
Phoenix is accessible by major bus and train lines and by interstate highways. Taxi service between Phoenix Sky Harbor International Airport and the Hyatt costs approximately $18-$25 one way.
