2008 AWARD FOR OUTSTANDING SERVICE
We want to meet your hero! Have you noticed a colleague whose practice is making a difference in the lives of homeless clients? Do you know a clinician who sacrifices his or her time to go beyond their job description to improve the health and quality of life of our homeless neighbors? Are you aware of a clinician who uses creativity and tenacious perseverance to address the challenges of homelessness in your community and beyond? Please join us in recognizing those individuals who serve homeless individuals in remarkable ways.
The Membership Committee of the HCH Clinicians' Network is accepting nominations for our Award for Outstanding Service until March 31, 2008. To submit your nomination, complete the 2008 Nominations Form. Please download both files page 1 and page 2. Both you and your nominees must be current Network members. With the presentation of this award, the Network strives to foster peer-to-peer recognition of the dedication and passion we see so frequently in hands-on care providers working with homeless families and individuals. It is these clinicians who labor tirelessly, but unrecognized, whom we hope to honor.
THE AWARD
The Network will present the Award for Outstanding Service during the opening plenary session at the 2008 National Health Care for the homeless Conference on Friday, June 13, in Phoenix, Arizona. The awardee must be present to receive the award, and the Network provides a travel stipend up to $200 to assure that the winner is able to attend the conference.
CRITERIA
- Nominees may include clinicians in a broad array of disciplines including nurses, social workers, outreach workers, oral health providers, physicians, nurse practitioners, substance abuse specialists, mental health providers, physician assistants, anyone providing hands-on care to homeless individuals and families
- Nominees must have advanced the companion goals of ending and preventing homelessness
- Nominees must be an individual who demonstrates vision and creativity in his or her work to improve the health and quality of life of homeless people
- The nominator must describe how and why you think this individual goes above and beyond his or her job description requirements
- Nominees must be clinicians who have worked with homeless people for at least three years
- Any current Network member may submit nominations; self-nominations are not accepted
- The nominee must be a current Network member
- Current Network Steering Committee members are not eligible for this award
APPLICATION PROCESS
To be considered for the 2008 Award for Outstanding Service, your completed Nomination Form and all accompanying materials must be received in the Network office no later than 12 noon central time, Monday, March 31, 2008. The nominations packet must include:
- the Nomination Form , which must be completed by the candidate's supervisor, colleague or other professional associate - download page 1 and page 2 (pdfs)
- a letter from at least one of the nominee's clients or patients describing how the nominee had a positive impact on his or her life
- a letter of support from a peer or co-worker of the nominee
- the nominee's resume or curriculum vitae
- a digital photograph [e-mail to bproffitt@nhchc.org] for display at the 2008 National Health Care for the Homeless Conference
SELECTION PROCESS
- A seven-member ad hoc committee convened by the Membership Committee Chair will consider evidence and documentation supports the information provided with each nomination.
- The selection committee will be comprised of representatives of the Network Membership Committee, the Network Steering Committee, Network members-at-large, and at least one previous Network award recipient.
- Network staff will notify all award nominators at least one month in advance of the National Health Care for the Homeless Conference.
